【ポイント】

◆胃癌治療ガイドライン第3版においては，Stage Ⅳ 胃癌を根治をめざした外科切除の対象外と位置づけている．

◆洗浄細胞診陽性例，軽微な大動脈周囲リンパ節転移陽性例，少数の肝転移を有する症例は例外的に切除の対象となりうる．

◆今後はStage Ⅳ 胃癌の治療方針についても，前向きの臨床試験でエビデンスを構築する必要がある．
