【ポイント】

◆非治癒因子を1つだけもつ症例に対するR0/1切除後の良好な治療成績が散見されており，切除可能なstage Ⅳ胃癌に対する外科的切除術を含む集学的治療の有効性が期待されている．

◆大腸癌でみられるように，胃癌においても腫瘍縮小効果の高い化学療法後のconversion surgeryを検討する価値があると思われる．他方，胃癌治癒切除後の補助化学療法ではS-1＋シスプラチンの忍容性が悪かったなど，外科切除によって化学療法が十分行えない危険性も十分考慮すべきである．

◆Stage Ⅳ胃癌は切除できたとしても再発率が高いという意味では，術後補助化学療法を含めて化学療法が治療の主役であり，術後化学療法の忍容性を意識して外科的切除術の適応は慎重に決定されるべきである．
