術後の門脈血栓・閉塞―治療のコツ

【ポイント】

◆消化器外科領域のなかでは，肝胆膵外科手術後に門脈血栓を経験することが多い．特に，肝胆膵領域への腹腔鏡下手術導入の拡大により，その頻度は増加傾向にある．腹腔鏡下脾摘術後には特に，高頻度に門脈血栓を生じると報告されている．

◆門脈血栓症の大部分は無症状で，画像診断の機会に偶然発見されることが多い．有症状の場合，門脈圧亢進症状から腸管虚血による腹膜炎まで多彩な症状を呈する．

◆門脈血栓症の治療の第一選択は抗凝固療法である．抗凝固療法が無効もしくは不能の症例には，病態に応じて血栓溶解療法，手術治療を選択する．
