【ポイント】

◆進行直腸癌に対する術前CRTの主目的は腫瘍学的予後の改善である．

◆自然肛門温存を目的とする術前CRTの役割は依然として確立しておらず，術前CRTを行った下部直腸癌術後の排便機能に関しては術式の違いもあって意見が分かれる．

◆術前CRT後の直腸癌手術では特有の有害事象に注意した手術や周術期マネージメントが大切である．
