食道胃接合部癌に対する外科治療戦略―組織型別の検討から

【ポイント】

◆縦隔リンパ節転移率は組織型によらず食道浸潤長に相関し，少なくとも#108以下の郭清が必要である．

◆腹部リンパ節転移率は組織型によらず高率であるが，#4d，5，6の転移頻度は低く，胃温存術式も許容しうる．

◆確実なproximal margin（PM）の確保（3.8cm以上）は重要な予後因子であり，pPM1回避には食道亜全摘も妥当である．

◆以上の条件を満たし安全に吻合が行える術式であれば開胸にこだわる必要はなく，経裂孔アプローチ，食道抜去，縦隔鏡下手術なども可能である．
