【ポイント】

◆食道胃接合部癌は胸腹部の境界領域に発生して多様な組織型を有することを特徴とし，わが国でも今後増加することが予測される．

◆近年のRCTの結果を基本としつつも，実際の腫瘍の進展範囲やリンパ節転移状況などを十分に把握したうえでの術式選択が肝要である．

◆縦隔リンパ節転移陽性例は予後不良であり，補助化学療法などを加えた集学的治療の必要性が示唆される．
