腹腔鏡下に摘出した脾臓sclerosing angiomatoid nodular transformationの1例

要旨

症例は49歳，男性．2年前に粘膜下層浸潤の下部直腸癌に対し内視鏡的粘膜下層剝離術（ESD）をされた．経過観察CTにて増大傾向のある脾腫瘍を指摘された．自覚症状はなかったが悪性腫瘍を否定できなかったため，腹腔鏡下脾臓摘出術を行った．腫瘤は脾臓下極から突出性に発育した60×60×60 mm大であったが被膜外への露出は認めなかった．腫瘍の割面は境界明瞭な白色充実性であり，辺縁部で結節性の出血斑が散見された．CD31，CD34，CD8の免疫組織化学的検査にてsclerosing angiomatoid nodular transformation（SANT）と診断された．本疾患は非常に稀な脾腫瘤形成性疾患であり，貴重な症例であると考えられた．本疾患のように脾臓の腫瘤形成性疾患では，診断的治療として腹腔鏡下脾臓摘出術のよい適応と考えられた．
