管外発育型を呈した巨大な回腸gastrointestinal stromal tumor（GIST）の1切除例

要旨

患者は62歳，男性．主訴は1週間前からの寝汗と微熱で，腹部症状の訴えはなかった．腹部造影CT，腹部造影MRIにて，長径20 cmを超える原発不明の巨大腹腔内腫瘍を認めた．開腹手術の結果，回腸末端より約110 cmの回腸から管外性に発育した，緊満した囊胞を主体とした不整な暗黒赤色の腫瘍を認め，小範囲の小腸切除で摘出することができた．病理検査の結果，GISTと診断された．再発率90%の高リスク群であったが，術後補助化学療法としてイマチニブ400 mg/日の36か月間の継続投与を重篤な副作用もなく行い，術後約4年2か月無再発生存中である．腫瘍径が大きく高リスクの症例でも，イマチニブ36か月間投与で長期無再発生存の期待ができることが示唆された．
