胃gastrointestinal stromal tumor（GIST）の経過観察中に発生した多発性胃GISTの1切除例

要旨

　症例は69歳，男性．2006年より胃体上部小彎側後壁寄りに1 cm大の粘膜下腫瘍を指摘され，それ以降2011年を除き毎年，内視鏡検査で経過観察していた．2012年10月の検査で，胃体中部小彎側に新たに2.5 cm大の粘膜下腫瘍を認めた．その急速な発育速度から高悪性度病変を疑い，胃部分切除術を行った．病理結果では顕微鏡的に確認された3個目の腫瘍を含めてすべてgastrointestinal stromal tumor（GIST）と診断された．胃GIST自体は稀な疾患ではないが，同時多発性の胃GISTの切除例の報告は稀であり，検索しえた限りでは4例目で，会議録を含めても14例目である．単発性が主である胃GISTも多発する可能性があることを念頭に，経過観察するべきであると考えられた．
