切除不能膵癌に対するdown-sizing chemo（radio）therapyの意義

【ポイント】

◆近年の化学療法の進歩にて，腫瘍や腫大リンパ節の縮小・消失（down-sizing）といった優れた治療効果が報告されている．

◆初診時は切除不能膵癌と診断された症例においても，化学（放射線）療法後に切除が可能となる症例もあり，このような手術は“adjuvant surgery”と呼ばれ，予後の向上が期待されている．

◆わが国において切除不能膵癌の定義は明確ではなく，切除率の向上および予後の延長を得るために明確な切除適応の判断と治療法のアルゴリズムの確立が早急に望まれる．
