【ポイント】

◆原発巣における手術切除範囲，センチネルリンパ節生検による腋窩郭清省略のために，癌の生物学的特性やエビデンスの理解が重要である．

◆新しい化学療法薬や分子標的薬により手術の縮小化・腋窩郭清省略の適応が広がる可能性がある．

◆転移・再発乳癌での再発巣切除やStage Ⅳ乳癌の原発巣切除は，局所制御の意義を十分に考慮して手術適応を検討することが望ましい．
