【ポイント】

◆胃切除のborderlineとして，①cM0でcStage Ⅲ以下であると考えられるが，切除に困難を伴うことが予想される場合，②cM1でcStage Ⅳであると考えられるが，M1の因子が限局していたり，前治療が奏効したりといった要因でR0切除が可能である/可能になったと判断される場合，が考えられる．

◆具体的には，①は（1）局所進行（T4b）症例，（2）大型3型・4型胃癌症例が，②は（3）M1因子が腹腔内洗浄細胞診陽性（CY1）のみである症例，（4）大動脈周囲リンパ節（No. 16a2, b1）転移症例，（5）一部の肝転移症例，といった疾患群が挙げられる．

◆補助療法の進歩により，胃切除のborderlineは変化しつつある．
