【ポイント】

◆肝細胞癌のうちborderline resectable症例は，肝障害度A，Bの高度脈管侵襲症例に対する肝切除，肝障害度Cのミラノ基準外に対する肝移植を検討する症例と考えられる．

◆肝細胞癌の高度脈管侵襲症例は積極的な手術治療で，TACE・肝動注療法・ソラフェニブ治療などよりも良好な予後を期待できる．

◆ミラノ基準外でも肝移植により良好な予後を示す肝細胞癌症例が存在し，日本全体のデータから新たな基準を策定すべきである．
