【ポイント】

◆膵癌は周囲脈管との関係を中心にborderline resectableが定義されることが多く，NCCNの定義が一般的に用いられている．

◆Borderline resectable膵癌に対する門脈合併切除はその有用性が示されているが，動脈合併切除は予後改善に結びつくかはっきりしておらず，その適応は慎重に判断すべきである．

◆術前化学療法の有用性が特に動脈浸潤例で報告されており，今後，臨床試験を通したエビデンスの確立が期待される．
