残胃の癌は減らせるのか—除菌について

【ポイント】
◆胃癌は組織型を問わずHelicobacter pylori感染に伴う慢性胃炎を背景として発症する．
◆胃癌治療には内科治療と外科治療があるが，内視鏡的粘膜下層剝離術の普及によって，胃癌の内視鏡治療の適応拡大がなされてきた．
◆H. pylori除菌が内視鏡治療後の異時癌の発生を抑制することは，わが国におけるランダム化比較試験より証明された．H. pylori除菌には異時癌の予防効果が期待できる．
◆外科切除後の残胃再発には，十二指腸液の逆流，迷走神経切離による脱神経支配など，術後の胃内環境の変化が関与しているが，H. pylori除菌の予防効果は明らかではない．
