■■はじめに
　完全腹腔鏡下胃切除は体腔内吻合を伴う腹腔鏡下胃切除で，腹腔鏡補助下手術と比べてさらなる創縮小効果，低侵襲性と，より確実な外科的切除縁が期待できる^1）．当科では，1997年より鏡視下手術の拡大視効果に着目して早期胃癌のみならず進行胃癌も含む治癒切除可能なすべての胃癌症例を対象とし，これまでに胃全摘を含む1,000例以上の完全腹腔鏡下胃切除を行い良好な成績を収めている^2,3）．
　本稿では，functional end-to-end anastomosis（FEEA）による幽門側胃切除術後Roux-en-Y再建（逆蠕動）の手技と，当科における体腔内吻合の原理原則について概説する．
