食道胃接合部癌に対するアプローチ：「経胸」の立場から―食道胃接合部癌における至適郭清範囲に基づく術式選択

　われわれがめざす普遍的な目的は，患者を根治へ導くことである．食道胃接合部癌の腫瘍学的見地に基づき至適郭清範囲を同定し，根治性を優先した術式を決定する必要がある．縦隔リンパ節転移は組織型にかかわらず解剖学的位置によるリンパ流に規定され，食道浸潤長が20 mmを超えるものには高い縦隔リンパ節転移リスクがある．これらには“右開胸食道亜全摘2領域郭清”が必要であり，縦隔転移を認めた例でも開胸手術による5年全生存率は47%で，長期生存は可能である．わが国においては右開胸食道亜全摘術の安全性は確立されており，リスク回避のために開胸を避ける必要はない．今後，根治性を最優先する立場に立って症例を蓄積し，より正確なリンパ節転移状況を把握し，真の治療方針を構築する必要がある．
