文献概要

　わが国では専門施設を中心に，下部直腸癌をおもな対象として側方郭清が行われてきた．近年の単一施設からの報告では，側方転移陽性例の5年生存率は36～42%である．側方リンパ節転移はStage Ⅳより予後は良好で，側方郭清が予後を改善することが，大腸癌研究会の集積データから明らかにされている．また，全直腸間膜切除と比べ，Stage Ⅲ直腸癌では生存率を改善することがメタアナリシスで示されている．側方郭清では，術中出血量の増加や術後の性機能・排尿機能障害が問題となるが，自律神経温存術の普及に伴い，これらの欠点は改善されている．

　一方，術前化学放射線療法は手術単独（全直腸間膜切除）に比べ，直腸癌術後の局所（骨盤内）再発率を低下させるが，側方リンパ節転移を制御できるという根拠に乏しい．また，生存率を改善しないことがメタアナリシスで明らかにされている．わが国においては，下部直腸癌に対する側方郭清は良好な生存率を担保する最も確実な局所療法と考えられる．
