下部直腸癌における側方リンパ節郭清：「化学放射線療法」の立場から

　下部直腸癌の局所再発抑制において，欧米の標準治療である術前化学放射線治療と，側方リンパ節郭清を直接比較した大規模第Ⅲ相ランダム化比較試験は存在しない．今までのエビデンスでは，局所再発制御には術前化学放射線療法が優っている可能性が高く，腫瘍縮小によるdown stage後，臓器温存の可能性や肛門括約筋温存の可能性においても術前化学放射線療法が優勢である．

　両治療法のデメリットとして性機能障害・排尿機能障害，手術操作の困難性，術後合併症の増加が挙げられるが，側方リンパ節郭清のほうが手術の侵襲が高く，これらのデメリットに関しても術前化学放射線治療のほうが軽度ではないかと推察される．しかしながら，根治手術まで時間がかかること，放射線治療における急性期・晩期障害や二次発癌の問題が術前化学放射線治療で劣る点である．今後の課題として，局所再発制御は可能となっても，最も重要な生存期間延長に寄与する報告が少ないことから，遠隔転移制御が予後を決定すると思われるため，化学療法のレジメン選択が重要と思われる．
