膵頭十二指腸切除における幽門の処理：「SSPPD；PrPD」の立場から

①胃排泄遅延の発生からみたメリット

　幽門輪温存膵頭十二指腸切除術（PpPD）では胃の貯留能は温存されるが，郭清に伴う迷走神経支配の喪失や血流の乏しくなった幽門輪の存在は胃排泄遅延の危険因子になる可能性がある．幽門輪のみを切除し，すべての胃を温存する幽門輪切除膵頭十二指腸切除術（PrPD：SSPPD）とPpPDとのRCTを施行した．胃排泄遅延の発生頻度はPrPD 3例（4.5%）（grade A 1例，B 1例，C 1例），PpPD 11例（17%）（grade A 6例，B 5例，C 0例）でありPrPDで有意に減少した（p＝0.0244）．
②SSPPD；PrPDを選択した場合のデメリット

　長期成績として術後2年間フォローし，晩期合併症，栄養評価（アルブミン値，プレアルブミン値）および体重変化を評価した．両術式ともに晩期合併症に有意差はなく，栄養状態および体重変化は同等であった．PrPDは長期成績である晩期合併症，栄養状態，体重変化においてPpPDと比較して遜色のない有効な術式であり，SSPPD；PrPDを選択した場合のデメリットはない．
