　Z0011試験，IBCSG23-01試験，AMAROS試験，これら3つのセンチネルリンパ節（SLN）陽性症例に対する追加郭清の省略を目的とした試験では，腋窩郭清省略（No-AD）群の無病生存率，全生存率は追加郭清（AD）群と同等であった．しかし，ADが局所制御に重要な役割を果たす症例は一定頻度存在する．BluteらのSLN陰性，腋窩リンパ節再発症例の検討では，腋窩再発後の5年無病生存率50%，5年全生存率58%と，決して良好ではない．上記3試験でも，no-AD群の腋窩再発率は有意ではないものの高い．また，SLN陽性後，追加郭清を省略したために，その後，腋窩再発を繰り返す症例をわれわれは経験している．ADのデメリットとされる上腕浮腫も，BMIが低い日本人では頻度は低く，許容できるレベルにあると考えられる．以上より，追加郭清省略は安全に施行できる症例群の特定など，さらなる研究の進展を待つべきであり，安易な導入は慎むべきである．
