画像上，完全に見えなくなった肝腫瘍に対する肝切除法―リアルタイム・バーチャル超音波装置とインドシアニングリーン蛍光法による手術ナビゲーション

はじめに

　近年，CTやMRIの画像精度の急速な進歩により，小さなサイズの肝癌や肝転移巣の描出が可能となった．また，第二世代の造影剤であるソナゾイド®を用いた体外超音波検査でも，より小さな肝腫瘍の描出が容易になった^1～3）．一方，このような小さなサイズの腫瘍は化学療法の著効により，画像上の完全消失，すなわちクリニカル・コンプリート・レスポンス（clinical complete response：以下，クリニカルCR）を示すことも報告されている^4,5）．それらクリニカルCR腫瘍は，たとえ術中造影超音波を用いても，その描出に難渋することがある．本稿では，クリニカルCR腫瘍に対し，リアルタイム・バーチャル超音波（real-time virtual sonography：以下，バーチャル超音波）装置と，インドシアニングリーン（indocyanine green：ICG）蛍光法を用いた肝切除法を考案したので紹介する^6～8）．

　なお，本手技の臨床応用は本学倫理委員会の承認を得ており，事前に患者および家族に説明し承諾を得たのち，本手技を施行している．
