術前に自然壊死を認めた肝細胞癌の1例

要旨

症例は57歳の男性で，近医にてC型慢性肝炎のフォロー中に超音波検査（US）で肝S5に径22 mm大の腫瘍を認め，PIVKA-Ⅱ 603 ng/dLから肝細胞癌と診断され当科へ紹介された．初診後に禁酒し，化膿性脊椎炎の治療目的で服用していたリファンピシン，ミノサイクリンは腎機能悪化のため当院2回目受診以降は中止した．手術直前のUSでは肝腫瘍は径12 mm大に縮小し，PIVKA-Ⅱは36 ng/dLと低下していた．自然退縮をきたした肝細胞癌と診断し，腹腔鏡下肝S5部分切除術を施行した．病理組織検査では腫瘍内部は壊死し，線維性被膜内に変性が加わった癌細胞が残存していた．
