【ポイント】

◆StageⅢA-N2はinfiltrativeとdiscreteに分類され，いずれの標準治療も化学放射線療法とされているが，後者には外科切除が加えられることが多い．

◆Salvage surgeryの報告は肺癌において極めて限られている．一方で，化学放射線療法後の手術は安全に施行可能であるとする報告もあり，最近のhigh-dose radiotherapy（74 Gy）に対する否定的な臨床試験の結果も手伝い，今後salvage surgeryの意義が再検討される可能性が高い．
