【ポイント】

◆サルベージとコンバージョンのterminologyについて述べる．

◆コンバージョンセラピー（adjuvant surgery）は，Stage Ⅳ胃癌に対する化学療法奏効中にR0をめざした手術を行うことで最大限の効果を引き出せる．

◆コンバージョンセラピーの意義を評価するには，化学療法の効果が得られた症例を化学療法継続群とadjuvant surgery群にランダム化した第Ⅲ相臨床試験が必要である．
