【ポイント】

◆サルベージ肝移植とは，肝機能の良好な肝細胞癌に対してはまず肝切除を施行し，その後経過中に肝細胞癌が再発，または肝機能が悪化したときに肝移植を行うものである．

◆サルベージ肝移植においては術後死亡率・再発率が高いとの報告がある一方，サルベージ肝移植と初回肝移植を比較し，術後合併症・長期生存率に差がなく，サルベージ肝移植が有用との報告も散見される．

◆サルベージ肝移植の適応基準については，ミラノ基準，UCSF基準，Up to Seven基準などで検討がなされているが，どこまで適応を拡大可能であるかはコンセンサスが得られていないのが現状である．
