【ポイント】

◆消化器外科で緊急手術を要する胆道疾患のほとんどは急性胆囊炎であり，その診断，手術適応を適切に判断することが重要である．

◆発症72時間以内の軽症・中等症胆囊炎に対しては早期胆囊摘出術が望ましく，腹腔鏡手術が第一選択として推奨される．

◆急性胆囊炎緊急手術では，待機手術に増して十分にcritical viewを展開し，胆管・肝動脈損傷や出血を確実に回避する．
