閉塞性黄疸の患者―術前の減黄処置は必要か

素朴な疑問

　閉塞性黄疸で発症した膵臓がん患者や胆管がん患者は，手術前に経皮的または内視鏡的に胆管ドレナージを行い，黄疸を消退させ肝機能障害を回復させておくことが必要である（減黄処置）．閉塞性黄疸を生じた患者に膵頭十二指腸切除を行う場合，減黄処置が不十分だと術後合併症が多いのだろうか．術前の減黄処置を行わないと手術は危険なのだろうか．
