食道癌術後難治性乳び胸に対して経皮経腹的リンパ管塞栓術で治療した1例

要旨

外傷や手術での胸管損傷により乳び胸などのリンパ漏を生じる．少量の場合は保存的治療も可能だが，多量の場合や保存的治療不応例では，栄養状態の悪化や免疫機能低下を引き起こし，時に致命的となる．保存的治療不応例には外科的胸管結紮術が行われてきたが，栄養状態不良であるため術後の死亡率は高く，胸管結紮後も乳び胸が継続する場合もあり必ずしも満足いくものではない．米国では1990年代後半より難治性乳び胸に対して経皮経腹的リンパ管塞栓術が行われており，良好な成績が報告されているが，本邦での報告はほとんど存在しない．今回われわれは食道癌術後に生じた難治性乳び胸に対して経皮的に胸管を塞栓し治療しえた1例を経験したので報告する．
