下部消化管縫合不全への対処法

【ポイント】

◆下部消化管縫合不全に対しては予防的な処置を講ずることが重要であるが，起こった場合の対処法には保存的療法と外科的療法がある．

◆診断には臨床・検査所見に加えて，ドレーンの性状の変化の確認や注腸造影などによる画像診断が有用である．

◆ドレーンによる縫合不全部からの消化管内容のドレナージが十分に効いていれば，栄養療法を含む保存的療法の適応となるが，急性に汎発性腹膜炎の状況を呈する場合や長期の保存療法に抵抗性の場合には手術療法が適応となる．
