門脈再建後の門脈狭窄・閉塞・血栓への対処法

【ポイント】

◆門脈切除再建後は門脈血栓を起こしやすく（10%），門脈血栓症は死亡率が高い（20%）．

◆術中に門脈系の屈曲・血栓に気づいた時点で外科的に対処し，また門脈狭小化の予防・早期発見に努める．

◆術後の門脈血栓はその程度にかかわらず，すみやかに内科的治療（抗凝固療法）を開始する．
