「大腸癌治療ガイドライン2014」改訂のポイント

はじめに

　わが国の悪性腫瘍の死亡率は年々増加の一途に歯止めがかからず，2012年には60年前の4倍となっている．その中でも，大腸癌の罹患数は年々増加を続けており，結腸癌の罹患数は3万2千人，直腸癌の罹患数は1万5千人を超え，部位別のがんの死亡率では女性で1位，男性で3位となっており，今後も増加の一途をたどると考えられる^1,2）．そのようなcommon diseaseともいえる大腸癌患者に対し，いわゆる専門施設であるhigh volume centerのみで賄うことは非現実的であり，全国の一般臨床医もそれぞれの役割を踏まえたうえで，診療に携わっていく機会がますます増えていると考えられる．そんな中，日本の大腸癌治療の施設格差をなくすことを目的の一つとして掲げている「大腸癌治療ガイドライン　医師用」の役割は年々重みを増していると考えられる．「大腸癌治療ガイドライン　医師用」は2005年度版を初版として，医療技術の進歩や新たな知見などに伴い，原則4年を目途に改定を行ってきた．本稿では，「大腸癌治療ガイドライン改定のポイント」と題し，旧版からの変更点を中心に総論および各論を概説する．
