【ポイント】

◆膵体尾部切除術の最終段階で，腫瘍が浸潤した門脈とのみつながった状態にするのが，門脈合併切除を安全に行うコツである．

◆門脈再建の原則は端々吻合であるが，切除する門脈長が長く吻合部に緊張が予想される場合は，左腎静脈グラフト間置を行う．

◆腹腔動脈合併切除は，膵尾・脾を後腹膜から脱転後，左からのアプローチを原則としている．
