周術期の血糖管理―インスリン療法は有用か

素朴な疑問

　手術患者や重症患者は耐糖能が低下して高血糖を呈することが多い．高血糖は易感染性・創傷治癒阻害・炎症反応亢進を生じ，術後合併症や回復遅延の原因になるため，インスリンを投与して高血糖を修正するが，血糖の目標値やインスリン投与法はさまざまである．手術患者や重症患者の厳しい血糖管理は，術後合併症の減少や転帰の改善に役立つのだろうか．
