【ポイント】

◆原因不明消化管出血（OGIB）とは，消化管出血が疑われた際，全身状態の把握・出血している消化管部位の推測・患者の正確な病歴聴取を行い，それらの情報をもとに上部・下部消化管内視鏡検査を行っても明らかな出血源を同定できないものを指す．

◆小腸出血をきたす原因疾患としては血管性病変のほか，びらん・潰瘍などの炎症性疾患，腫瘍性疾患，または憩室などが挙げられる．

◆カプセル内視鏡は全小腸の観察能に優れており，バルーン内視鏡より低侵襲でスクリーニングに適した検査といえる．バルーン内視鏡は全小腸を一度に観察することは困難であるが処置が可能である．しかし検査時間は比較的長く，苦痛を伴うこともある．
