【ポイント】

◆クローン病（CD）において絶対的手術適応となるのは，炎症が増悪して深掘れ潰瘍を呈し，穿孔や穿通に続く膿瘍，あるいは大量出血の原因となりうる場合である．また，内科的治療に反応せず炎症の増悪・寛解に伴い腸管の創傷治癒を繰り返した場合には瘢痕性狭窄や瘻孔形成をきたし，相対的な手術適応となる．

◆再手術率が高く，短腸症候群が問題となることから，CDの小腸病変では可及的に腸管を温存することが重要であり，狭窄形成術または小範囲切除が行われる．

◆再発予防として内科治療ではメサラジンやニトロイミダゾール系抗菌薬，プリン誘導体，また近年は抗TNF-α製剤であるインフリキシマブの有用性が示されており，禁煙の重要性も報告されている．
