術前胆道ドレナージ術

【ポイント】
◆急性胆管炎の有無で緊急胆道ドレナージ術の必要性が決定する．
◆胆管癌の発生部位により，最適な術前胆道ドレナージ術の種類を使い分ける必要がある．
◆遠位胆管癌の場合は，プラスチックステントを用いた内瘻術が基本となる．術前待機期間が長期間の場合は，メタリックステントの留置も考慮する．
◆肝門部領域胆管癌の場合は，原則として，片側肝葉（残存予定肝）のみの内視鏡的経鼻胆道ドレナージ術で対応する．
