【ポイント】
◆R0切除は治療成績の根幹にかかわる．MDCTを含めた画像診断技術の向上により，R0切除率・生存率の上昇が認められている．
◆胆管癌におけるMDCTの役割は大きく，減黄前に適切なMDCTを撮影できれば，進展度診断から切除術式の立案まで可能である．肝臓の3D画像は残肝容積の評価・切離ラインの理解に有用であり，主要脈管の3D画像は立体解剖の直感的把握，手術シミュレーションに適している．
◆減黄の際に必須となるERCPに引き続いてIDUSを行うことで，より正確な進展度診断・胆管の立体解剖の把握が可能となる．
