【ポイント】
◆進行胆道癌においては，R0切除を行うためには拡大肝右葉切除，左三区域切除などの広範囲切除が必要となる．
◆閉塞性黄疸例における広範囲肝葉切除（60%＜）の安全性を確保するためには，術前門脈塞栓術（PTPE）が必須である．
◆画像診断により癌の進展範囲，解剖学的変異の有無および肝予備能（ICGR₁₅値），残存予定肝容積を考慮し，適切な術式を選択しPTPEを行う．
