胆管癌切除の肝十二指腸間膜剝離時のnon-touch isolation

【ポイント】
◆肝十二指腸間膜の左側を切開し，肝動脈とその背側の門脈をテーピングする．
◆肝十二指腸間膜から肝動脈と門脈を引き，胆管を含む周囲結合織をテーピングする．腫瘍を直接把持しない．
◆遠位部胆管癌の右肝動脈浸潤では右肝動脈切除を要するが，門脈浸潤がなく肝門板が温存されれば動脈再建の必要はない．
