【ポイント】
◆胆管癌に対する肝膵切除（HPD）の最大の課題は，安全性と癌の根治性を両立させることである．
◆正確な腫瘍進展範囲診断は重要であり，術前処置として胆道ドレナージと門脈塞栓術を行う．
◆術後膵液漏の重症化予防が手術の安全性の向上のために必須のストラテジーである．
◆広範囲胆管癌に対するHPD術後の遠隔成績は比較的良好であり，積極的に試みられるべき術式である．
