術後補助化学療法

【ポイント】
◆当科でのゲムシタビンとS-1（GS療法）を用いた進行胆道癌に対する術後補助化学療法の適応は，胆道癌取扱い規約第6版でのStageⅠ症例には再発例を認めなかったことから，Stage Ⅱ以上の胆道癌を適応としている．
◆当科における206例の進行胆道癌切除例の検討では，GS療法を用いた術後補助化学療法施行は，独立した予後規定因子で有意に生存率が良好であり，同治療は進行胆道癌切除症例の生存率を向上させる可能性があることが示唆された．
◆現在，わが国において胆管癌切除症例を対象としたゲムシタビンおよびS-1による術後補助化学療法の有効性を評価する第Ⅲ相無作為比較試験（BCAT，JCOG 1202 study）のほか，いくつかの術後補助化学療法に関連する臨床試験が進行中であり，胆道癌の罹患率の高いわが国から全世界規模で新たなエビデンスのある補助療法を発信していくことが期待される．
