胃外科における消化管吻合のコツ—手縫い

【ポイント】
◆消化管吻合の要点は縫合不全を生じないこと，そして吻合部狭窄をきたさないことである．
◆手縫い吻合の利点は消化管壁の各層を適切に接着できる点であり，縫合では縫い代，縫い幅，糸の締め具合の3点に注意する．
◆吻合に際してはできる限り自然な縫合と結紮が行えるように，術者と助手でゆとりのある場を作ってから開始する．
