文献概要

　膵頭十二指腸切除術（PD）は，1935年にWhippleらが報告して以来，様々なアプローチ方法，血管合併切除・再建法，消化管再建方法などの改良を加えながら確立されてきた．しかしながら，術後合併症発生率は40%（特にGrade B，Cの膵液瘻の発生率は13.2%），術後30日以内の死亡率は1.2%，入院死亡率は2.8%と，他の消化器外科手術と比較していずれも高いのが現状である^1）．特に，局所進行膵癌では，血管合併切除を伴った拡大手術が必要となることが多いが，血管走行や癌の進展範囲によっては，術者の多彩な経験や高い技術が要求される．また，PD特有の合併症として膵液瘻が挙げられるが，それを回避すべく各施設で様々な取り組みが行われている．
　本稿では，PDにおける血管処理，膵空腸吻合におけるバリエーションをいくつか提示し，標準術式とともにその対策について解説する．
