■■はじめに
　手縫いによるBillrothⅠ法（以下，B-Ⅰ）は，幽門側胃切除後の再建方法として，本邦では従来標準的な手技であり，普遍的に行われていた^1）．近年，腹腔鏡下手術の普及に拍車がかかり，さらなる低侵襲性を目指す流れのなかで，B-Ⅰを完全体腔内で行うためのさまざまな吻合法が開発されてきた^2〜5）．それらはすべて自動縫合器を駆使したものであり，背景には腹腔鏡下手縫いの煩雑さを回避し，簡便化を図る目的があった．このトレンドに対し筆者らは，開腹手術で支持されていた手縫い吻合の長所（単純さ，柔軟さなど）に着目し，腹腔鏡下にもこれを実践している．
