■■はじめに
　自動吻合器（サーキュラー・ステイプラー）による食道空腸端側吻合は，長年にわたり多数の施設で受け入れられ，標準的に行われている再建方法である^1,2）．食道断端へのアンビルの埋没固定法としては，開腹手術においては巾着縫合が一般的であるが，腹腔鏡下ではその煩雑さを回避する目的で，他の方法が開発され普及している^3,4）．あるいはサーキュラー・ステイプラーを用いない吻合法も多く行われている^5,6）．
　筆者らはサーキュラー・ステイプラーによる食道空腸吻合を行っているが，食道壁全層の確実な巾着縫合を可能にする手縫いの利点^7）に着目し，腹腔鏡下でもこれを実践している．開腹手術では食道断端部は視認不良となる場合があるが，腹腔鏡下では良好な視野で捉えられるため，精緻な巾着縫合が可能となると考える．本稿ではその手技の実際を供覧する．
