当院での腹腔鏡下脾臓摘出術の検討—術後門脈血栓症の治療と予防について

要旨
［目的］腹腔鏡下脾臓摘出術の安全性と問題点を検討した．［対象］2008年4月〜2013年9月に当科で施行した腹腔鏡下脾臓摘出術（用手補助下8例を含む）19例の治療成績につき検討した．［結果］術後出血や膵液瘻などの術後合併症は認めなかった．門脈血栓症（PVT）が5例（26.3%）に発症したが，全例とも保存的治療で軽快した．PVT発症例の摘出脾臓重量は，PVT未発症例より有意に大きかった（257 g vs. 435 g，p＜0.05）．［結語］腹腔鏡下脾臓摘出術は定型化された標準術式であるが，巨大脾腫症例においては術後PVT発症のリスクを念頭においた周術期管理が必要である．
