胃原発炎症性筋線維芽細胞性腫瘍の1例

要旨
症例は54歳の男性で，腹部膨満感を主訴に受診した．腹部CT，MRIにて腹腔内に最大径12 cmの境界明瞭な囊胞性病変を認めた．由来臓器は明らかではなかったが，腫瘤は可動性があり，大網もしくは腸間膜に由来する病変が疑われた．大網囊腫の術前診断で手術を施行した．病変は胃大彎から発生しており，大網，横行結腸と癒着していた．癒着を剝離したのちに胃局所切除を行い，腫瘤を摘出した．術後経過は良好で，術後7日目に退院した．病理組織学的検査では腫瘤に悪性所見は認めず，胃原発炎症性筋線維芽細胞性腫瘍と診断された．本疾患は診断確定が困難であるが，局所再発の可能性が指摘されており，定期的な経過観察を要する．
