GEP-NETの腫瘍概念の変遷と本邦診療ガイドラインについて

【ポイント】
◆OberndorferがNETを①境界明瞭，②転移せず，③成長緩慢で有意の大きさに達せず，④良性である，と記載してカルチノイドと名付けた．その後，リンパ節転移，肝転移をきたすことが明らかになった．
◆NETは①機能性か非機能性か，②遺伝性疾患に併存しているか否か，を鑑別して，③Ki67指数の測定をして，④全身を精査したうえで治療することが重要である．
◆NETガイドラインは文献の精査と専門家の討論の結果作成されたが，NETは希少疾患に属していて，種々の治療法の予後に対する効果が科学的に証明されていない．
◆日本神経内分泌腫瘍研究会が2014年末からNET患者登録事業を始めた．ここに多くの患者が登録されれば，未解決の臨床的課題の解決に向けて前進できる．診察したら，登録をお願いしたい．
