腹腔動脈合併尾側膵切除術中に肝動脈再建を併施した局所進行膵体部癌の1例

要旨
症例は50歳，女性．膵体部癌と右卵巣癌の重複癌に対して化学療法を施行後に，腹腔動脈合併尾側膵切除術と両側卵巣切除術を予定していた．しかし，術中迅速病理組織検査で固有肝動脈，胃十二指腸動脈にまで膵癌の浸潤を認めた．腫瘍の遺残がない手術とするために固有肝動脈，胃十二指腸動脈を合併切除し，右総腸骨動脈を中枢吻合部とするバイパスによる肝動脈再建を併施した．両側卵巣切除術の予定であったことからgraftには両側の卵巣静脈を吻合したcomposite graftを使用した．術後24か月経過した現在，無再発で生存しており，graftの狭窄や血栓の形成を認めず肝血流は良好に保たれている．肝動脈再建の方法として右総腸骨動脈を中枢吻合部とするバイパスは有効な方法である．
